Our staff will measure your dancer for their costume during class time. However, we would like some additional information to help us make costume decisions. Please complete the following section to the best of your knowledge.

Refund Policy*

I understand that recital fees are non-refundable. If you reverse your submission before the registration period closes, we can apply a credit to your account. If you opt out of the show after 1/28/25 you forfeit your performance fee. No credits will be issued after 1/28/25.

Dress Rehearsal*

MANDATORY DRESS REHEARSAL - I understand that if we miss the dress rehearsal, we will not be allowed to participate in the recital. You only need to attend the rehearsal you are assigned to.

Recital Schedule*

THERE WILL BE NO EARLY ASSIGNING OR PROMISING OF SHOW PLACEMENT FOR ANY REASON WHATSOEVER. - Due to the high volume of students we cannot take requests to perform on either Sat or Sun. We have to make a schedule/line up based on a number of factors. Personal schedules cannot be one of them. All dancers should block off Wednesday through Sunday until the lineups are announced. (Excluding Friday for Juneteenth)

I understand we cannot make requests for our assigned recital date.

Attendance Policy*

I understand that attendance leading up to the recital is required. Missing an excessive number of classes or dropping from the program during the final 6 weeks before the show day will lead to being dropped from the performance without a refund.

Committing To Your Group*

I understand that committing to this recital makes us part of a team. The individual classes all work on a choreographed routine. Dropping out of the recital at any point, particularly late in the season, will have a significant negative impact on the class. Please follow through on your commitment.

Payment required by deadline to hold spot*

I understand that payment of the fees guarantees my spot in the recital. If payment is not received by the deadline PCPA will not order a costume for you and we will not hold your spot.

Keep Account Current*

We are open to and happy to offer payment plans or financial support if need be.

I understand the Principal Arts Directors reserve the right to withdraw my child from the show should I carry an outstanding tuition balance on my account. All billing must be current to add further commitment, fees, and performance privilege.

Release of Liability*

I agree to hold harmless Small Fry Dance Company LLC and Principal Creative and Performing Arts, its associates, teachers, and owners from any liability associated with providing dance lessons and live stage performance events for my child. I agree that photos taken during classes and/or performances may be used anonymously in promotional material.
